An incident is a real-world event that has already occurred. This article explains the difference between a violation and an incident, and walks through how to log, view, and review incidents in Horizon.

What is an incident?
A violation is a potential compliance issue that requires review. An incident is different: it's a real-world event that has already occurred.
Examples of incidents: A vehicle accident. A workplace safety incident. A data breach. A theft event.
Incidents are logged manually by your team and linked to the relevant risk, so they're tracked alongside your compliance record.
Log an incident
Important: Incidents cannot be modified after they've been entered. Make sure you have all the necessary information on hand before logging an incident.
- Navigate to the Risk Register.
- Select the relevant Risk ID.
- Select the Incidents tab.
- Click Create new incident.
- Enter the following information:

| Title | A short, descriptive name for the incident. |
| Description | A summary of what occurred. |
| Incident Type | Select from the drop-down menu. See Incident types below. Alternatively, toggle on Enter type manually to type a custom incident type. |
| Member | The workforce member involved. Search by name. |
| Date Occurred | The date the incident took place. |
| Date Reported (optional) | The date the incident was reported. |
| Was reported by (optional) | The person who reported the incident. Search by name. |
- Click Save.
Incident types
Horizon includes the following predefined incident types:
- Fraud
- Conflict of interest
- Data breach
- Agreement breach
- Theft
- Bad behaviour
If none of the predefined types apply, toggle on Enter type manually to enter a custom incident type instead.
View logged incidents
To view all incidents logged against a risk:
- Navigate to the Risk Register.
- Select the relevant Risk ID.
- Select the Incidents tab.

What's the difference between a violation and an incident?
A violation is created automatically by Horizon when a rule is broken. It's an early warning that action is needed. An incident is a real-world event that has already occurred, and is recorded manually by your team.
Do I have to select from the predefined Incident Types?
No. Toggle on Enter type manually to enter a custom incident type if none of the predefined options apply.
Are Date Reported and Was reported by required?
No, both fields are optional when logging an incident.
